To give our suppliers access to the most up-to-date forms and information we are providing you with the following links to the US Customs and Border Protection website (www.cbp.gov) and the Canada Border Services Agency (www.cbsa-asfc.gc.ca).

Feel free to contact either Trade department. For the US, contact the SAF-HOLLAND USA office in Muskegon, MI or for Canada contact the SAF-HOLLAND Canada office in Woodstock, ON for assistance regarding these forms.



SAF-HOLLAND USA, Inc. - Contractor Information Packet

SAF-HOLLAND USA, Inc. - requests that our contractors and/or subcontractors have the following information on file in our facility before they begin to work on-site for any SAF-HOLLAND USA Facility. Be aware that you will not be allowed in our facilities until the following documentation is received:

- Signed Contractor Acknowledgement Form (EMS 4.4.2)
- Current Certificate of Liability Insurance that is up to the level of coverage requested
- Evidence of Safety Programs in effect for your workers to meet OSHA and State regulations
- Completed W-9 form
- When appropriate, a full unconditional lien waiver or partial unconditional lien waiver may be requested before final payment is approved and if it is deemed necessary at the time of service.
